Submit my video

Steps to submit a video:

Complete the online submission form below and press submit – only one form required per video.
Print off and sign the 2013 Care to Change Release Form – each team member MUST sign a form.
Burn your video to a disc.
Mail or hand deliver the Release Form plus the video disc to:
